We are a mobile healthcare provider, which means we can travel directly to you! Whether you are a homebound adult or senior who prefers to have home medical services, our friendly and experienced nurse practitioner provides you with compassionate care and addresses your needs from the comfort of your home or assisted living facility. No more waiting rooms or rushing through appointments. We take time to listen to you then develop a plan designed to help you feel better and improve your overall quality of life.
Professional
AWW Health Connection, LLC collaborates with professionals to ensure the needs of our patients are addressed. We set up referrals for mobile medical services such as wound care, ophthalmology, and podiatry. Our nurse practitioner also coordinates home health care services such as physical therapy, occupational therapy, and speech therapy. We also facilitate radiology services such as x-rays, ultrasounds, and echocardiograms. Our nurse practitioner orders bloodwork and other tests through a mobile laboratory and can make referrals for specialty telemedicine medical services, including psychiatry.
